The registration form is 5 (five) pages long. It needs to be turned in by the first day of camp. Registration forms are kept on file and only need to be completed once, unless there is a change in information.

You have several registration options:

• Fax your enrollment form with credit card payment
• Mail your form and payment to Cool Ventures for Kids at the IcePlex
• Email your enrollment form with credit card payment
• Bring your form and payment to the IcePlex office. Mon- Fri. 7am - 7pm.

• 100% credit/refund for IcePlex cancellation
• A credit will be given for money already paid if cancelled 14 days or more prior to camp. $25 Deposit will not be credited. No Refunds are given.
• $25 Deposits are nonrefundable
